Print or download an evaluation from a previous academic year

To access your annual faculty activities list from a previous evaluation year in Faculty180, please follow these steps:

  1. Click on FORMS & REPORTS located at the bottom of the left pane (scroll down on the left pane if necessary). A new page will appear on the main area of the screen.
  2. Click on Prior Annual Faculty Activities Forms located on the main area of the screen (not the left pane).
  3. Click on the blue action eyeball for the respective form that you would like to view. For example, you can click on “SPC Faculty Evaluation: Summer 2015 – Spring 2016”
  4. The form will be displayed on your screen.

To access your Dean, Chair, or Program Director’s notes from a previous evaluation year in Faculty180, please follow these steps:

  1. Click on EVALUATIONS located on the left pane. A new page will appear on the main area of the screen.
  2. Click on the blue action eyeball (located on the right side of the page) for the desired item. For example, you can click on “Annual Evaluation 14-15” under the Evaluations column (not on left pane but on main area). A new and similar page will open.
  3. Click on the blue action eyeball for the desired item (located on the right side of the page). For example, you can click on “Annual Evaluation 2015-16” under the View/Respond column to access the Dean/Chair/Program Director’s notes from that year.
  4. Your evaluator’s notes will be displayed on your screen. From here, you can also access the classifications form by clicking on either Dean Evaluation of Categories or Evaluate Adjuncts on Sections. Note: Deans are not required to leave notes on adjunct evaluations.

To print: Once the desired section has finished loading on your screen, hold down the Ctrl key then tap the P key on your keyboard to open up the printing dialog box. If you are using a MAC computer, hold down the Command key and then tap the P key on your keyboard.

To download as PDF: Follow the same process to print, but instead of printing, select to print as PDF. If your computer doesn’t have the option to print as a PDF, please contact TRS to have it installed.

If you require a signed paper copy of your evaluation, please consult your Department Dean.
